Medicare Advantage vs. Medicare Supplement (Medigap): How to Compare

Compare Medicare Advantage and Medigap on costs, doctor choice, and extra benefits, and learn why timing matters in North Carolina.

Last updated October 2026

‍

The Short Answer: Neither option is best for everyone. Medicare Advantage plans often have low premiums and extra benefits but use networks and charge copays. Medicare Supplement (Medigap) plans work with Original Medicare, cost more each month, and cover most out-of-pocket costs with any doctor who accepts Medicare. In North Carolina, switching to Medigap later may require health questions, so timing matters.

‍

The basics

Medicare Advantage (Part C) is an all-in-one alternative to Original Medicare offered by private insurance companies. It covers everything Original Medicare does and usually includes prescription drug coverage and extra benefits like vision, dental, hearing, etc.

Medicare Supplement (Medigap) is extra insurance you add to Original Medicare. It helps pay costs Original Medicare does not, like some deductibles and coinsurance. It does not include drug coverage, so most people also buy a stand-alone Part D plan. It also does not include extra benefits like vision, dental, hearing, etc. Typically, premiums are higher and increase every year. You cannot have both at the same time.

Things to think about

Your health and expected care

If you see specialists often or expect a major procedure, a Medigap plan's low costs at the time of care may matter more. If you are generally healthy, lower monthly premiums may matter more. Think about both your good years and your harder years.

Your doctors

With Original Medicare and Medigap, you can see any provider who accepts Medicare. With Medicare Advantage, check that your doctors are in the network each year. Hopscotch accepts both.

Your budget

Compare your total yearly cost: premiums plus what you might pay when you get care, up to the plan's maximum.

Your ability to switch later

Your best time to buy a Medigap plan is your Medigap Open Enrollment Period: the six months starting when you are 65 or older and enrolled in Part B. During that time, insurers must sell you a plan regardless of your health. After that window, insurers in North Carolina can generally ask health questions and may charge more or deny coverage, except in certain situations called guaranteed issue rights. That means leaving Medicare Advantage for Medigap later is not always possible.

One exception: if you joined a Medicare Advantage plan for the first time when you became eligible for Medicare, you may have a "trial right" to switch to Medigap within your first 12 months.

‍

If you have both Medicare and Medicaid

If you qualify for full Medicaid, it usually helps pay your Medicare costs, so a Medigap plan is often not needed. Dual Eligible Special Needs Plans (D-SNPs) are Medicare Advantage plans designed for people with both. One of the trusted brokers we work with can help you understand your options.

Frequently asked questions
Is Medicare Advantage or a supplement better?
It depends on your health, doctors, budget, and how much flexibility you want. Medicare Advantage often costs less each month but uses networks and copays. Medigap costs more each month but covers most costs with any provider who accepts Medicare.
Can I switch from Medicare Advantage to a Medigap plan?
You can return to Original Medicare during Annual Enrollment (October 15 to December 7) or the Medicare Advantage Open Enrollment Period (January 1 to March 31). But outside certain protected situations, a Medigap insurer may ask health questions and can deny you or charge more.
Does Medigap cover prescriptions?
No. Medigap plans sold today do not include drug coverage. Most people with Medigap also enroll in a stand-alone Part D plan.
Can I have Medicare Advantage and Medigap at the same time?
No. Medigap only works with Original Medicare, and it is illegal for anyone to sell you a Medigap plan if you are in a Medicare Advantage plan (unless you are switching back to Original Medicare).
