Our approach to chronic care
The average senior patient has three or more chronic conditions. Traditional primary care practices often aren't designed to manage all of them. Disconnected care and unclear records make it harder to treat the whole person. Hopscotch was designed differently from the start, with a focus on prevention, a care team you can reach out to at your convenience, and resources that go beyond the clinic walls.
A note on primary care vs. specialty care: Hopscotch provides primary care, which is the ongoing management and coordination of your health. For specialized procedures or conditions requiring a subspecialist, we'll refer you and remain actively involved in your overall care. You'll never feel like you're navigating it alone.

This list reflects the most common conditions our providers manage, but it's not exhaustive. Primary care providers at Hopscotch are trained to evaluate and manage a wide range of health concerns. If you're unsure whether we can help, the best thing to do is reach out. We'll be honest about what we can manage and what might require a specialist.
